Controlled Substances Renewals

How Often?  All registrations must be renewed annually.
Late Renewal Penalty: $25 (Applied on November 1 and May 1 for registrations that expire on October 1 or April 1)

Renewal Applications

Applications for renewal are mailed out 45 days prior to the expiration date, to the last address listed in our database. If you did not receive a renewal application, please call our office at 803-896-0636 or 803-896-0635 to request one. Please Do not use the ‘New Application’ forms elsewhere on this website as it will not be processed.

MDs, DOs and EMS Units: Renewal applications for these registrations are mailed during the third week of July. Registrations must be renewed prior to October 31, or a late fee will be added. If not renewed by December 31, the registration will be canceled.

DMD, DDS, DVM, OD, APRN, PA-C, Pharmacies and All Other Registrants: Renewal applications for these registrations are mailed during the third week of January. Registrations must be renewed prior to April 31, or a late fee will be added. If not renewed by June 30, the registration will be canceled.

What If My Registration Expires?

An expired registration that is canceled by DHEC without any correspondence from the registrant, due to failure to renew said registration by the registrant, cannot be reinstated unless a satisfactory explanation is provided, in writing, and a penalty of $100 is paid.

Note: Section 166(b) of Regulation 61-4 (South Carolina Controlled Substances Regulation) requires a registered person to notify DHEC’s Bureau of Drug Control and request a renewal application, in writing, if he or she does not receive a renewal application within 45 days before the expiration date of his or her registration.
