If you are a vendor selling new or used X-ray equipment or providing X-ray related services in South Carolina, you are required to register with DHEC. X-ray equipment and services vendors are regulated under Regulation 61-64, X-Ray (Title B), Rules and Regulations for Radiation Control.

X-Ray Vendor Registration Forms

• Vendor Registration Form (pdf) (DHEC 0824) Use this DHEC form to register a business that provides services for and to X-ray producing machines.
• Vendor Employee Registration Form (pdf) (DHEC 0825) Use this DHEC form to register individuals who provide services for and to X-ray producing machines. Every employee who provides services for and to X-ray producing machines must register with DHEC.

Once you register with DHEC, make sure to file monthly sales reports.
