Compliance: Privacy Complaints

Before filing a complaint with the South Carolina Department of Health and Environmental Control (DHEC) Compliance Office, please read all the information below to ensure your complaint is filed with the appropriate authority. If a complaint is not within our area of responsibility you may be referred to another federal, state, or local agency as appropriate.


Privacy Complaints

You may file a privacy complaint with DHEC if you believe that DHEC has violated the privacy or security of health or other confidential information. Please note that complaints for violation of the Health Insurance Portability and Accountability Act (HIPAA) must be filed within 180 days of the time you become aware of the violation.

Filing a complaint

To ensure that we have the necessary information, you may complete and submit the complaint using one of the methods below. If you have additional supporting documentation, you may submit that information along with the complaint.

  1. Online – Complete and submit the Privacy Complaint Form.

  2. Mail/Fax - Type or print all information clearly and send the Privacy Complaint Form (PDF) to

  3. Telephone - If you are unable to file online, by mail or fax, or otherwise have questions then call the DHEC Privacy Officer at (888) 843-3718 to file a complaint.
